2023 Mabuhay, Inc. Youth Awards Nomination Form – Deadline for submission is on August 6, 2023 at 11:59 P.M. No late submissions will be accepted.
Mabuhay, Inc. seeks to recognize Youth members who have shown exemplary academic performance while rendering community service in and outside the activities and programs of our organization for School Year 2022-2023. This includes extracurricular activities done in school, like cheerleading, band/music, dance team, peer tutoring, and the likes.
Kindly submit one nomination form for each nominee. Please also email supporting documents (i.e. electronic copies of report cards, awards and honors, graduation diploma, etc.) to Austin Conaty at austin.conaty@gmail.com and Moks Laureles at moks.laureles@mabuhayinc-md.org to qualify for this year’s Mabuhay, Inc., Youth Awards.
