Youth Awards Nomination Form
Each year, Mabuhay, Inc. honors graduating students and young individuals who have made meaningful contributions through academic achievements, community service, leadership, and participation in extracurricular activities during the 2024–2025 school year.
Selected students will be recognized in person at the 2025 Mabuhay, Inc. Youth Awards,
held during the Annual Picnic on Sunday, August 17, 2025, at
Holy Family Catholic Church, 12010 Woodmore Road, Mitchellville, MD 20721.
Nomination Requirements:
- Nominee must be a current Mabuhay, Inc. member.
- Nomination and its supporting documents must be submitted by 11:59 P.M. on August 10, 2025. Incomplete nominations and late submissions will not be accepted.
Examples of supporting documents include diploma, report cards, awards, certificates of recognition, etc.
If preferred, mail photocopies of supporting documents to the address below.
However, it must be postmarked no later than August 8, 2025.
